A Polish passport application is completed only on the basis of Polish civil status records. Every detail — given names, surname, place of birth, the mother’s maiden name — has to match the Polish birth certificate and, where relevant, the Polish marriage certificate. If those Polish records do not exist yet, the passport cannot be applied for at all.

The passport is collected only at the consulate where the application was filed. It cannot be forwarded to another post or to Poland.

Payment is taken at the consulate in cash, by money order, cashier’s check or card. Personal checks are not accepted anywhere in the network. Reductions apply to students aged 18 to 26, pensioners, people with disabilities and large-family card holders, on production of the document proving it.

How to renew

  1. Check that your Polish civil records exist and that your data matches them.
  2. Book a free appointment through the consular booking system.
  3. Prepare a biometric photograph to the Polish specification.
  4. Attend in person. The officer completes the form electronically and you confirm it on a signature pad.
  5. Pay at the consulate by an accepted method.
  6. Collect the passport at the same consulate once it arrives from Poland.

Additional notes

The 120-day rule after a name change

When a surname changes and the national register is updated, the old passport becomes invalid automatically 120 days later. After that point the reduced replacement fee no longer applies either, so act quickly.

Long-expired passports

If your Polish passport has been invalid for well over a decade, the consulate may require a citizenship confirmation procedure before accepting a new application.

Children born in the United States

A child’s U.S. birth certificate must be transcribed into the Polish civil register first. Only then can a passport be requested, and the identification number is assigned at that appointment.

Photographs

Polish photograph rules differ from U.S. ones on framing and background, and a non-compliant photo stops the appointment.
